Sparrow recently announced plans for the Population Health Services Organization, a new service model designed to help Physicians manage the care and well-being of entire populations while care is coordinated across the health care system.
As at Sparrow, Population Health puts the needs of the Patient first. Population Health Management emphasizes a higher level of coordinated services to better manage the chronic conditions of the highest risk Patients. The PHSO will provide support to standardize care practices using best practices, help Caregivers work to their maximum skill level, and provide data analytics to inform the work of Caregivers and support Patients.
Much has been accomplished over the last few months to move Sparrow further toward this goal. A Care Model Steering Committee has been established, and we are now building the necessary internal expertise in data analytics and health informatics that will enable us to implement these new strategies and interventions, and to assess their progress and effectiveness.
Stay tuned for further developments later this summer as Population Health Management helps Sparrow to enhance the delivery of care across the mid-Michigan region, drive growth, and challenge conventional structure in order to succeed in the new era of health care.
